No payment is required to submit this form. You will receive a secure link to purchase your test referral after our Clinical Nutritionist reviews this submission.

I understand and agree to the following:

  • The results of the Microba report will only be released after completing an Initial Consultation with Health Connection's Clinical Nutrition practitioner, Ellen Winnett (BHSc), because this is a practitioner-referred test. Standard consultation fees and clinic availability applies.

  • An Initial Consultation costs $180.00 AUD, and lasts 60 minutes.

  • You will receive an email or SMS with the referral code or link to purchase the test directly through Health Connection Clinic, subject to our practitioner approving your request. There is no mark-up on test prices, and your practitioner does NOT receive a commission from referring Microba's tests.

  • You will receive the test kit via express post after payment is processed.

  • Results are available to your Health Connection practitioner 28 days after your sample is received by the Microba lab.

  • You will receive an email from Health Connection Clinic once the results are with your practitioner, as well as an invitation to book an Initial Consultation if you haven't already.

  • You reside in Australia.
